I'm not sure what their rates are like for dogs, but I have my cat on the best plan ($50 deductable per condition per year, 100% coverage, $20k/year limit) and it's something like $275/year. The price does vary by state (when I lived in MA it was well above $300/year for the lowest plan).
My kitty turned out to be a little sicky, so I've racked up thousands and thousands of dollars in vet bills in the 4 years I've had her. It's really saved my cookies.
